CONSENT TO PARTICIPATE IN SURVEY

 

I understand that I am being asked to participate in a survey being conducted by the Canadian Network for Arts and Learning (CNAL).

 

A. PURPOSE

 

I have been informed that the purpose of the survey is as follows:

 

To gain a better understanding of the impact of Arts Can Teach programs on participating artists, teachers, and students.

 

B. PROCEDURES

 

I understand that the following procedures will be followed:

 

  • Responses will be analyzed and shared through written reports with Arts Can Teach.

 

C. CONDITIONS OF PARTICIPATION

 

  • I understand that I am free to fully withdraw my consent to participate at any time before, during, or after the survey. 
  • I understand that my participation in this survey is CONFIDENTIAL (i.e., the researchers will know, but not disclose your identity).

 

  • I understand that the data from this research may be published.


 

I HAVE CAREFULLY STUDIED THE ABOVE AND UNDERSTAND THIS AGREEMENT.  I FREELY CONSENT AND VOLUNTARILY AGREE TO PARTICIPATE IN THIS RESEARCH.
