I understand that I have been asked to participate in a survey being conducted by the Canadian Network for Arts and Learning on behalf of Arts Can Teach.
A. Purpose
I understand that the purpose of the research is to gain a better understanding of participants’ experience with The Arts Can Teach program, and their impressions of its effectiveness, with a view to improving and enlarging the program.
B. Procedures
The survey will ask a number of rating scale questions. My answers will be aggregated with those of other respondents to produce a numerical summary of responses.
The survey will also provide 2 opportunities for me to provide written responses. My written comments will be analyzed along with those of other respondents to provide an overview of respondents’ perspectives.
I will be asked to identify my occupation. My name will not be collected. My participation in this research is confidential.
I understand that the data collected in this study may be published.
C. Conditions of Participation
Participation in this study is completely voluntary. If I decide not to participate there will be no negative consequences. I may stop participating at any time and I may decide not to answer any specific question.
